RENTAL APPLICATION ADDENDUM "A".
RENTAL APPLICATION REQUIREMENTS, CRITERIA, DEFINITIONS, AND QUALIFICATIONS.

ALL PERSONS ARE ENCOURAGED TO APPLY FOR ANY AND ALL RENTALS SUPPLIED BY MADISON

The only rental agreement for any rental terms, payments or agreements will be offered solely by Madison Real Estate & Property Mgmt., Inc.

Madison Real Estate & Property Mgmt., Inc. is a Washington State licensed Real Estate Company. This company, by Agency Law, is bound to work and rent all Real Estate for the best interest of the owners.

Rental Applicant Requirements

1. FULLY COMPLETED RENTAL APPLICATION, ONE (1) PER ADULT** APPLICANT. MARRIED COUPLES COMPLETE ONE (1) APPLICATION. Adults requiring a live-in guardian/supervisor or 24 hour care giver(s): the live-in guardian/supervisor or 24 hour care giver(s) must fill out an application. A Reasonable Accomodation Request form is included with this application
2. $40.00 CERTIFIED FUNDS, APPLICATION FEE is NON-REFUNDABLE and payable when application(s) is remitted to Madison Real Estate & Property Mgmt.., Inc., to complete the application process.

THE FOLLOWING ITEMS MUST BE PROVIDED FOR EACH ADULT LISTED ON THE APPLICATION. (Copies of these items can be made in the office upon remittance of the application(s).
1. COLOR COPY OF CURRENT DRIVER LICENSE, STATE ID, AND/OR MILITARY ID for each applicant.
2. VERIFIED INCOME SOURCES; Employer recent paystubs/W2/I-9. New hires/transfers please provide a letter of hire/transfer. Award letters/bank deposit history: for Social Security/SSI and/or pension. Financial Aid/Scholarship letter(s), unemployment stubs, etc. Self-employed individuals must provide their last two (2) years income tax returns to verify self-employment income. Child support may be considered as income with documented proof of payment /collection.
VERIFIED NON-INCOME SOURCES; Education Grants, Food Stamps, Loans, gifts, etc.

CRITERIA FOR APPLICATION APPROVAL

Applicant(s) screening report will be reviewed for the following items and/or adverse (negative) information:.
* CREDIT HISTORY: Civil judgments and/or collections amounting to over $1000 and/or more than 10 collections, past rental and/or utility obligations, OPEN bankruptcy and a minimum Beacon score** of 475. Debt to income ratio more than 75%.
* COURT RECORDS:History of criminal behavior. OPEN felonies, felonies closed under 7 years ago, misdemeanors- drugs, sex offense, theft, assault, DUI/DWI, etc. CRIMINAL CONVICTIONS MAY DISQUALIFY APPLICANT(S).
* RENTAL REFERENCES**: Current and/or prior continuous, positive & objective rental history. Lack of/or negative and/or incomplete rental references.
* EMPLOYMENT HISTORY**/INCOME: Current employment of 3 months or more. Inconsistent or lack of proper documentation proving adequate income to pay rent. Earnings must be at least 2x the monthly rent.
* SERVICE ANIMALS: SERVICE ANIMALS ARE WELCOME. You must supply certified doctor orders & accreditaion certificate for each animal.
* NON-SERVICE ANIMALS: DOGS, OVER 20LBS, AND CATS MAY DISQUALIFY APPLICANT(S). No dangerous breeds allowed. Applicant may be required to provide proof of insurance & may incur additonal cleaning costs.
DEBT TO INCOME RATIO: Debt to income ratio is calculated by adding the monthly rent plus the total amount of your debts. This total is divided by your total income. This amount should be no greater than 75% of the applicants gross income.
* COMPLETE AND SIGNED APPLICATION: All adult applicants must sign and date this sheet, the application and the Reasonable Accomodation Request form. Incomplete and/or illegible applications will not be processed.

If any of the criteria listed above is not met, a letter of explanation for extenuating circumstances will be accepted with all applications. In such cases the management company may approve an applicant at it's sole discretion and may offer one or more of the following options: ask for a qualified co-signer**, increase the rent, ask for one or two months additional rent, and/or increase the deposit at the time they are given possession of the property.

A GENERIC RENTAL AGREEMENT MAY BE REVIEWED IN THE MANAGEMENT COMPANY OFFICE MONDAY THROUGH FRIDAY DURING REGULAR BUSINESS HOURS.

DEFINITIONS

ADULT:  ANY PERSON EIGHTEEN (18) YEARS OF AGE OR OLDER. ALL PERSONS OVER THE AGE OF 18 ARE CONSIDERED A LEGAL AGE ADULT AND MUST FILL OUT A SEPARATE APPLICATION EVEN IF THEY ARE DEPENDENTS OF ADULT APPLICANT(S).
BEACON SCORE:  A RATING SCORE USED BY ACARNET CREDIT INFORMATION SERVICES TO PREDICT THE LIKELIHOOD THAT A POTENTIAL CREDIT CUSTOMER WILL BECOME A SERIOUS CREDIT RISK WITHIN THE NEXT 24 MONTHS.  SCORES RANGE FROM 350 TO 950, THE LOWER THE SCORE, THE HIGHER THE POTENTIAL CREDIT RISK.
EMPLOYMENT HISTORY:  A MINIMUM OF THREE (3) MONTHS WITH CURRENT EMPLOYER OR HISTORY OF EMPLOYMENT IN SIMILAR POSITIONS.
POSITIVE RENTAL REFERENCE:

·     RENT REGULARLY PAID ON TIME, NO LATE RENTS
·     TENANTS GAVE PROPER NOTICE TO VACATE
·     POSITIVE LANDLORD COMMENTS, I.E.; WOULD RENT TO AGAIN, TOOK CARE OF PROPERTY, NO PROBLEMS GOOD TENANTS, ETC.
·     NO NEGATIVE LANDLORD COMMENTS
·     NO TENANT CAUSED PROPERTY DAMAGE
·     RESIDENTAL ADDRESSES SHOWN ON APPLICANT'S CREDIT REPORT THAT ARE NOT DISCLOSED ON THE APPLICATION WILL BE QUESTIONED AND MAY HAVE AN ADVERSE EFFECT ON THE APPLICANT'S RENTAL APPROVAL.
QUALIFIED CO-SIGNER:  Adult relative (Father, Mother, Grandparent(s), Brother, Sister) who owns Real Property in the state of Washington and meets the Management Company's income and credit requirements as set forth herein.

QUALIFYING CRITERIA

THE FOLLOWING ITEMS MAY/WILL AUTOMATICALLY DISQUALIFY AN APPLICANT IF EXPLANATIONS ARE NOT PROVIDED WITH THE APPLICATION:

·     IRS, STATE, OR COUNTY LIENS AGAINST APPLICANT
·     DSHS CHILD SUPPORT LIEN AGAINST APPLICANT
·     FELONIES, DUI/DWI CONVICTION(S)
·     10 OR MORE UNPAID COLLECTIONS TOTALING $1000.00 OR MORE
·     DEBT TO INCOME RATIO ABOVE 75%
BANKRUPTCY MUST BE FULLY DISCHARGED: If applicant continues to have unpaid delinquent accounts or more than 5 late pays after the bankruptcy is discharged, their application is subject to denial.

APPLICANTS HAVE THE RIGHT TO DISPUTE ANY ADVERSE INFORMATION OBTAINED.
* In compliance with Washington State's Fair Tenant Screening Act of 2012 and the Fair Credit Reporting Act (FCRA) this is to inform you that the background investigation will be processed by Acranet the Information Network. We may be obtaining credit reports, court records (civil and criminal), arrest detention information, employment and rental references as needed to verify all information put forth on your rental application.If you are denied due to information obtained through Acranet, you have the right to request a free copy of your credit report from Acranet the Information Network, 521 W.Maxwell/ P.O. Box 5393, Spokane, WA 99205-0393 (509) 324-1350 or 1-800-845-7435. You have the right to dispute, directly with the credit reporting agency, the accuracy or completeness of any information reported by the credit reporting agency.
* If you are denied due to information obtained from third party sources, you have the right to obtain disclosure of the nature of the information by making a written request to our office with 60 days of your application being denied.
* You have the right to obtain a FREE copy of your credit report from every credit bureau; Equifax, Experian & Trans Union. A FREE copy or your credit report may be obtained once a year from www.annualcreditreoprt.com.

FALSE/MISLEADING AND/OR OMISSION OF INFORMATION OF FACT OR LACK OF INFORMATION PROVIDED OR AN INCOMPLETE APPLICAITON MAY DISQUALIFY TENANT(S).
